Optum

Optum is the health-services engine of UnitedHealth Group, combining clinical care delivery, pharmacy benefit management, and healthcare data analytics into a single unlisted division. Revenue reached USD 270.6 billion in FY2025, up from USD 155.6 billion in FY2021, making it larger than most standalone healthcare companies. Optum operates through three segments: Optum Health, with over 2,200 care sites and 20 million direct clinical patients; Optum Rx, a pharmacy benefit manager connected to more than 67,000 retail pharmacies; and Optum Insight, which sells revenue-cycle and payment-integrity software to hospitals and payers. It employs roughly 300,000 people and is headquartered in Eden Prairie, Minnesota.

Strengths: an integrated model that links insurance, care delivery, pharmacy, and analytics, allowing UnitedHealth to capture margin across the patient journey rather than at a single point. Optum Rx handles pharmacy spend for tens of millions of members, Optum Health's value-based clinics supply predictable patient volume, and Optum Insight's data assets are difficult for competitors to replicate.

Weaknesses: the division is wholly dependent on UnitedHealth's balance sheet and strategy, and its growth by acquisition has drawn regulatory scrutiny. Following UnitedHealth's 2025 profit decline to USD 12.1 billion, management scaled back what it called the division's rapid expansion, and Optum Rx has been shifting to a fee-based PBM model under pressure over transparency.

Optum is the clearest example in American healthcare of vertical integration executed at scale: an insurer that owns the clinics, the pharmacy counter, and the software that administers claims. Its growth story is exceptional on paper, but 2025-2026 brought a correction, with UnitedHealth's profit falling and Optum's aggressive expansion explicitly rolled back.

Industry Position

Optum competes on three separate fronts. In care delivery it runs against CVS Health's Oak Street Health and Signify Health, Humana's CenterWell, and thousands of independent physician groups. In pharmacy benefit management it faces CVS Caremark, Cigna's Express Scripts, and Elevance's Carelon. In healthcare IT and revenue-cycle management its rivals include Oracle Health, Epic Systems, Waystar, and McKesson. What distinguishes Optum is that it participates in all three simultaneously, so a health plan contracting with Optum Rx may also buy analytics from Optum Insight and send members to Optum Health clinics - a bundling strategy competitors cannot easily match.

Competitive Advantages Group revenue rose from US$155.6 billion in FY2021 to US$270.6 billion in FY2025.

Data is the core asset. Optum Insight processes claims and payment data for 9,000 hospitals and health systems and hundreds of payers, which gives the group visibility into cost trends before they appear in reported results. Optum Health's 4 million fully accountable value-based patients provide a captive base for clinical innovation, and SCA Health's ambulatory surgical centres capture procedures moving out of hospitals. The pharmacy network of 67,000 retail outlets and Genoa's behavioural-health pharmacies extend reach into community settings, while UnitedHealth's capital allows acquisitions that smaller competitors cannot finance.

Strategic Expansion The group employed roughly 300,000 people at 2026-01-27. Principal brands include Optum Health, Optum Insight, Optum Rx, Optum Financial.

The strategic emphasis has shifted from acquisition-led growth to integration and transparency. Optum Rx moved to a fee-based PBM model in 2026, decoupling revenue from drug-price spreads in response to employer and legislative criticism. Optum Health is consolidating its clinic footprint after rolling back rapid expansion, focusing on markets where value-based contracts already exist rather than open new geographies. International operations in India, Ireland, the Philippines, Brazil, and Colombia continue to supply lower-cost technology and service capacity, and Optum Serve retains federal contracts that give the division a government revenue channel independent of commercial insurance cycles.

Risks & Outlook Operationally it runs Over 2,200 clinical care delivery sites, ambulatory surgical centers (via SCA Health), urgent care clinics (via MedExpress), and on-site behavioral health pharmacies (via Genoa Healthcare), connected to more than 67,000 retail pharmacies and 90,000 aligned physicians across all 50 U.S. states, alongside global technology and operations centers in India, Ireland, the Philippines, Brazil, and Colombia..

Regulatory risk is the dominant concern. Vertical integration between a payer and its own care and pharmacy arms invites antitrust attention, and UnitedHealth's settlement with the FTC over insulin pricing in 2026 shows the willingness of regulators to pursue the group. Medicare Advantage rate pressure affects both the insurance parent and Optum Health's value-based contracts. The transparency-driven shift in PBM economics reduces a long-standing margin source, and the departure of Optum's former chief executive adds leadership uncertainty. On the other side, an ageing US population, the migration of care out of hospitals, and the group's data position make demand for its services structurally strong. Balancing scale and integration against regulatory and reputational pressure, Optum earns a VerityRank Score of 86/100. Named peers include CVS Health (Caremark / Oak Street Health / Signify Health), The Cigna Group (Evernorth / Express Scripts), Elevance Health (Carelon).
